Thank you for helping to feedback to the Office of the Service Complaint Ombudsman (OSCO).  Please help us by taking a few minutes to complete this short questionnaire and give us your views on our website.

We are keen to gather feedback on your experiences with the OSCO and establish how we can work to continually improve the service we provide.

The form should take no longer than 1 minute to complete.

The responses you give will be used to:
  • monitor our performance;
  • shape future improvements;
  • ensure our users consistently receive an excellent level of service.
All responses are entirely anonymous, you will not be asked for your name or any identifying information.  The responses will be held and analysed by OSCO statistical only, in line with our commitment to the rights of access to information which are set out in the Freedom of Information Act 2000 and the Data Protection Act 1998.
