Thank you for taking time to complete this survey to let us know the difference the Macmillan Horizon Centre has made for you. This survey is designed to gather feedback based on your experience to help us understand how we are doing and how we can improve. We estimate it will take you 15-20 minutes to complete this survey. If you need any further support or you would like to find out more about the Macmillan Horizon Centre please contact us at horizoncentre@macmillan.org.uk or call 01273 468770. We have designed the questions to be anonymous, however there are open text boxes. We ask you not to leave any information in there by which we can identify you such as your name or the dates when you used our services. If we unintentionally collect data by which we can identify you, we will destroy this and not use it as part of our survey results. We intend to keep the results for three years and no longer. This survey is hosted by Smart Survey but is controlled by Macmillan Cancer Support.
