We want to make Memory Walk 2019 the most successful yet – we need to raise vital funds to continue to fund research, campaign for change and to offer support for people affected by dementia when they need it most.

By sharing your story you'll be helping us to raise vital awareness of dementia and encourage others to take part in Memory Walk.

We will not use your story or pass the details recorded in this survey to any other organisation without your permission. The information will be stored on our database for a maximum of three years.
