Welcome to this Surrey Heartlands Primary Care Engagement survey. 

As part of our co-design work to help address the current challenges around primary care access, we are launching this survey to get your feedback about the tools (such as online consulting, SMS etc) that support remote access. We'd also like to understand more about the changes / improvements that would help you support patients more effectively, both within the practice and at scale.

The areas we're considering (when talking about 'remote consulting') are the following:
SMS (121, supporting remote consulting)
Questionnaires 
Batch SMS (bulk messaging, appt reminders)
Online consulting
Video consulting
GP websites
Facilitation of eHubs / Collaboration Hubs and at scale working across the local network of providers



We will be bringing the feedback from this survey along with the subsequent work in the co-design process into a set of requirements to take into procurement. This will help us make sure we have the right tool(s) to most effectively meet our needs into the future. We will be doing this work collaboratively with primary care.
