Sexual Health Services: Service Users and Non-Users Consultation

Introduction

We would like to find out more about what people want from sexual health services in Warrington.  We would be grateful if you would complete this short 10 minute questionnaire.

The survey is anonymous and what you tell us will be treated as confidential, you cannot be identified by your answers.

The information that you supply on this survey is subject to the provisions of the Data Protection Act 1998. The information you provide will only be processed for the purpose for which it has been given and will not be used for any additional purposes without your consent.

All of the information you provide through this survey will be analysed and kept completely confidential and secure by Warrington Borough Council.

When we publish results, we do not publish individual details or data only aggregate information/overall results (apart from written comments, where given, which always remain anonymous).
