Thank you for expressing interest in joining us to talk about the next phase of the Airedale Hospital rebuild. These first workshops begin our latest round of involvement.
We want to listen to a range of people who might have different experiences when using the hospital, and different views about what’s important. Therefore, we are asking people to tell us more about themselves.
The broader the range of people we have in the workshops, the richer the conversations we will have, and the better the information we can feed back to people leading the project.
Please complete the following form by Friday 27 June. You don’t have to answer every question, but we encourage you to share as much as you feel comfortable sharing.
