Evidence for Learning (Personal): I give permission for my child to be photographed by themselves for their learning journals.
Personal Care: I give permission for my child to be supported with personal care such as toileting/washing hands/ getting dressed and undressed for swimming/PE
Medical Information: I give consent for the school nursing team and the school to share relevant information about my child/young person.
Evidence for Learning (Other): I give permission for my child to appear in other children’s learning journey photographs e.g. as part of a group or in an activity, learning alongside each other.
Internal Displays: I give permission for photos of my child to be used in class and wider school displays, curriculum handbooks and materials.
Website: I give permission for my child’s photo to be used on the school website.
Social Media: I give permission for my child’s photo to be used on the school social media accounts.
School Charity: I give permission for my child’s photo to be used for school charity promotion.
