We would be grateful if you could complete this this short survey to understand your experience with diabetes services in GP practices. We would like to understand the gaps in the diabetes service from a patient perspective, and to try to identify opportunity for improving patient experience.
 
The aims of the survey are:
  • To understand the Diabetes service provision in GP practices
  • To build service for the future that focus on prevention, detection and protection
  • To improve the patient experience by better linked GP and community diabetes services
