This form should be completed as fully and accurately as possible and submitted within one school day of the permanent exclusion. Incomplete forms will not be accepted - the information is vital for the safe and efficient admission of pupils to alternative provision by the sixth day. 
​Members of the Independent Review Panel (IRP) will have regard to the Devon Ladder of Intervention when considering any application for review, as should school Pupil Disciplinary Committees (PDCs). If a school has failed to follow the Ladder process, the IRP will question the school's reasons for considering permanent exclusion as the "last resort".
