--Title of Project: Whether parental perceptions of the perceived benefits and barriers contributes towards participation rates of children with disabilities within sporting activities --Name of Researcher and School/Faculty: My name is Ellise Wooley and I am a sports coaching student from John Moores University. I believe when speaking to parents it is highly important to hear your view when discussing a specific topic. I am inviting yourself to take part in a research study that I am investigating. Before you decide, it is important that you know why I am doing this research and what it involves. Please take time to read the following information. Ask myself if there is anything that is not clear or if you would like more information regarding a specific question. Take time to decide if you want to take part or not. 1. What is the purpose of the study? The purpose of the research is to find out what your own view as a parent is on children who has a disability involvement in sport. I am doing this investigation as I have a passion for children to be part of sport activities. Even more so children who have a disability taking part in sport. I believe there are various aspects that can be improved and benefitted due to the children taking part in health aiding activities. 2. Do I have to take part? No. It is completely up to yourself on whether you want to take part in the study. If you would like to help myself to develop key research in regards to answering the research question. You are able to depart from the research at any time and without giving any reason to myself. A decision to withdraw will not affect your rights/ any future treatment/service you receive. 3. What will happen to me if I take part? When taking part in the study, not much of your time will be taken and the information taken from yourself will be highly beneficial for myself. There are very basic steps to follow and these are displayed: -Information of the research -Reading information sheet -Completeing questionnaire -Returning questionnaire The period of time you will be involved in the research is however long you make it, it could take you 10 minutes or could take you maximum 30 minutes to complete the questions. You will be asked to complete a short questionnaire including 15 questions. Once this is complete that will be the only action you will need to do. When taking part in the research it will only last as long as 30 minutes maximum to fill out the short questionnaire. 4. Are there any risks / benefits involved? When taking part in the research no risks, discomforts or inconvenience will be included when taking part. 5. Will my taking part in the study be kept confidential? When taking part in the study, all information will be kept confidential and safeguarded during and after the study. All information will be stored safe and only be accessed by myself through passcode until sent to examiners. The information will only be kept for the maximum amount of time needed and will be deleted once the report has been assessed and completed. -- Contact Details of Researcher To contact myself in regards of returning questionnaires simply complete the questionnaire on the questionnaire link and I will recieve the results automatically.
