The South Downs National Park Authority (SDNPA) has appointed Cordis Bright to carry out a review of social prescribing activity in and within a 10-kilometre radius of the South Downs National Park. The overall purpose of our review is to understand more about the different forms of social prescribing activity currently taking place and people’s views about the current capacity within the sector and limitations on it. This research will help SDNPA to understand how it can best support health and wellbeing providers to develop their offer so that more people can benefit from access to the National Park for health and well-being. 
 
This survey is aimed at: 
Organisations providing a social prescribing service (for example, you employ link workers) and 
Groups and organisations offering activities or services that are or could be used by people referred through social prescribing.  
 
The purpose of this survey is (a) to collect factual information about what your organisation does in order to compile a spreadsheet containing details of the social prescribing services on offer in the South Downs National Park and (b) to seek your views on what support you may need, how your services could be developed and where there are gaps in support for health and wellbeing.  
 
By completing this survey, you will be consenting to having factual information about your organisation (Qs 1-16) included within our list of services. This will be shared publicly, excluding any contact names and details.  
 
Your views on the support you may need and how the social prescribing offer in the South Downs National Park could be improved (Qs 17-26) will be reported anonymously and the data will not be shared with other organisations.
