Survey for people living with dementia and their family and friends

The London Borough of Bromley and Bromley Clinical Commissioning Group are undertaking a review of the Dementia Hub and Dementia Respite at Home Service to help develop the services.

We want to make sure that the services meet the current and future needs of people with dementia and their family and friends.  Through this survey we hope to get feedback from people who use the service to aid us in developing key areas.

Please help us shape the new service by giving us your views.

Thank you for taking the time to complete our survey.

We will use the information you provide to improve the resourcing and delivery of services and to comply with our statutory and legal obligations.  Responses to the survey are confidential and no personal information is relayed to those who directly provide services for you.   Information about how the council uses and protects personal data and about personal data rights is available at www.bromley.gov.uk/privacy
