Introduction
 
Thank you for taking part in this short survey. It should take 10-15 minutes to complete. To thank you for your time you will have the option of being entered into a free prize draw to win a tablet.
 
This survey has been designed by Citizens Online, an independent digital inclusion charity. We have been commissioned by Derbyshire County Council to research digital exclusion and work with organisations in the County who are concerned about the problem to develop a strategy and action plan.  
 
One of the organisations we are working with is Rural Action Derbyshire (RAD). We've designed this survey with them as their digital inclusion project has similar aims; to ensure that no one in Derbyshire is left behind in the digital world we now live in. 
 
RAD is currently developing a digital inclusion project that aims to: map the 'network' of digital support across Derbyshire, spot gaps in digital support, and work with partners and local communities to fill these gaps by piloting new digital inclusion activities and support individuals who would like to become Digital Ambassadors and Digital Champions within their communities. 
 
All of your responses are confidential and will help us to understand how your organisation uses technology and supports others to use technology (or would like to).

1. I understand that my participation is voluntary. 
2. I agree for my answers to be analysed together with others' responses, and published as part of the research project. 
 
The results will be presented to Derbyshire County Council, Rural Action Derbyshire and shared with their stakeholders. For the purposes of this survey, the term customers, clients, users and participants are all encompassed in the term - service users.
