Introduction
An Education, Health and Care (EHC) plan is a legal document that describes how children's education, health and social care needs will be met, to help them achieve their agreed goals.
This questionnaire is for parents or carers of children and young people who have received an Education, Health and Care (EHC) plan.
Norfolk County Council wants to hear about your experience of getting an EHC plan and will use the information you give to understand how the process of getting an EHC plan can be improved.
Because we want you to complete this survey anonymously, you will not be asked to give any information that will disclose you or your child's identity. It will therefore not be possible to identify anyone who has completed the survey and all responses will be confidential and only used to support the local authority in its improvement journey. All data is stored in a password protected electronic format – please read our privacy notice for further information as to how your data is used and your rights.
All completed surveys are collated into a single report giving a collective perspective on how the EHC plan process is working in Norfolk.
By completing this survey we are assuming that you are giving your active consent for us to use the information you provide to help us identify what is working well and what needs to be improved.
How to answer the questionnaire
Please answer the questions in relation to your child, or the child you care for who has an EHC plan.
There are no right or wrong answers. It is important that you answer all questions, but you can choose not to take the survey (see question 1 below), to stop responding at any time, or to ignore any questions that you do not want to answer (after question 1).
