Introduction

Warrington's Public Health team would like to understand more about how we can continue to support our partners and colleagues to strengthen and improve the health of their workforces.  In order to do this we would like to ask you some questions about your current approach to workplace health and what you feel would be of benefit to your organisation in its efforts to maintain the health and wellbeing of your staff.  This questionnaire should take no more than 15 minutes to complete and your contribution to this work would be sincerely appreciated.

We comply with all legislation governing the protection of personal information, including the General Data Protection Regulation (GDPR) and the Data Protection Act 2018. Any personal information you supply will remain strictly confidential and anonymous and will be held and used in line with the Data Protection Act 2018.
