This campaign has now finished, but we always want to hear from you, about the services you have used and what you thought of them. You can tell us what you think about doctors, dentists, mental health services, chemists, hospitals - any health or care service that you've used, on our feedback form.

Healthwatch Northumberland is YOUR local NHS and social care champion.

We'd like to know how you're feeling and hear what you think about NHS services. This could be doctors, dentists and mental health support. This feedback form is for young people aged 13-24 years old. It will only take a few minutes to complete and you don't have to answer any questions you don't want to. By taking part you have the chance to win a £100 Love2Shop voucher and if you go to a school or college it might win a prize too! Closing date 28 April


We will produce a report based on what we hear. This will be shared with NHS leaders and other decision makers to help make local health care better for you and other young people
