Healthwatch Windsor, Ascot and Maidenhead is your local health and social care champion. As an independent statutory body, we have the power to make sure NHS leaders and other decision makers listen to local feedback and improve standards of care.

The results of this survey will be collated and put into a report which will be shared with the Local Authority and Local NHS  and will be published on the Healthwatch Windsor, Ascot and Maidenhead website. All responses will be anonymised and no one will be able to be identified by the experiences they share. The report will be used to inform the way services develop.

As well as your feedback, we also ask you to volunteer some personal information. This helps us to help us understand how different groups experience local health and social care services and supports our focus on improving equality, diversity and inclusion.
