Why is this form necessary?

​Corsham Institute makes recordings to share and evidence our work. We may use any images we take of your child on our associated websites, social media channels or in printed publications, as well as use any videos or audio recordings of your child on our associated websites or social media channels. From time to time, we may also provide any recordings captured to media outlets, which may mean they appear on third party websites or social media channels, local or national newspapers, or on televised programmes.

This release form provides us with your permission to use the content we create from photographing, filming or recording your child, meaning that we can publish it.
Please read the points below and tick the box at the end of this section to confirm your agreement:
  • The photographs, video or other recordings made will be processed and stored in accordance with the UK Data Protection Act 2018 (EU General Data Protection Regulation 2016/679).
  • You freely give your permission for materials filmed, photographed or otherwise recorded of your child to be used on our associated websites, social media channels, printed publications or by third party media outlets for marketing or publicity purposes.
  • This recording will be freely available on the Corsham Institute website and may be linked to other publicly available internet sites, as we share our work to further our cause.
  • We may edit the photo, video or recording as we see fit, however, if you object to the edits made, you have the right to request that we promptly correct or permanently delete your child's contribution.
  • Any opinions stated by your child in the recordings are your child's own opinions, any facts expressed by your child are, to the best of their knowledge, true and nothing your child says is defamatory or infringes any other right of any person.
  • You consent for us to keep the photo, video or recordings of your child for a period of up to five years from the date indicated on this form, unless we have other reason to remove it. At the end of this five year period we will delete any photo, video or recordings of your child we hold, but they may continue to appear in publications already in circulation.
  • You can ask Corsham Institute (by contacting data@corshaminstitute.org) to:
- Confirm to you what personal data it may hold about your child, if any, and for what purposes; 
- Change or withdraw your consent that has been provided to Corsham Institute at any time;
- Correct any inaccurate or incomplete personal data which may be held about your child; 
- Provide you with a complete copy of your child's personal data for you to move elsewhere; 
- Stop processing your child's personal data, whilst an objection from you is being resolved; 
- Permanently erase all your child's personal data promptly, and confirm to you that it has done so (there may be reasons why we may be unable to do this); 
- Stop using your child's content at any time, in which case it will not be used in future publications but may continue to appear in publications already in circulation.

This agreement shall be governed by and construed in accordance with English law.
