Thanks for coming to sign up your workplace to take part in our Spring into Action Workplace Challenge.

This challenge is for workplaces in Norwich and Great Yarmouth.

To register, we will need you to provide some basic information (detailed below) about your workplace, through this short survey.

• Name of your workplace
• Postcode for your workplace
• Number of employees based at your workplace
• Contact information for your workplace

Please note that sign up to this challenge is by workplace rather than organisation so if your organisation has multiple sites in Norwich and Great Yarmouth you will need to complete a separate form for each workplace.
