This short survey has been designed collaboratively by Patient Voice @ Strand [PPG], Practice Management Team and GP Partners and is asking for your experience over the last 12 months with the Strand Medical Surgery.
The survey is being hosted by Healthwatch West Sussex in order that there is independent and unbiased analysis of the insight shared by patients.

Anything you share will remain anonymous. You give your views freely and all information will be held confidentially by Healthwatch West Sussex.
The feedback you share will help your surgery team and PPG to identify areas where improvements can be made and assess the current quality of services based on patient experience.
A full report, based on all the feedback received, will be produced by Healthwatch West Sussex and shared with the practice and all patients, in due course.
Following the feedback from the survey, Healthwatch will be working with the practice team to develop an action plan, ensuring the views and experiences of patients influence future plans and improvements.
