Waveney Dementia Projects Grants application form

A recent workshop identified opportunities to improve provision for people with dementia and their carers across Waveney District. Funding to enable this has been made available through the Improved Better Care Fund via Suffolk County Council and Great Yarmouth and Waveney Clinical Commissioning Group (CCG).
 
The funding is for:

• Projects that clearly address one or more of our priorities – see Section 3
• New projects to support people with dementia or projects that extend / grow existing activity
• Projects that provide local solutions to local needs
• Grant allocations will be up to a maximum of £15,000
• Up to 100 per cent of project costs but we welcome projects that have match funding 
• You are more likely to receive a grant if you:
• Are clear about exactly how the grant will be spent
• Can demonstrate a clear need for the project
• Can explain the difference that it will make
• Can show what your organisation has achieved so far – your track record
• Are actively involving people with dementia and their carers
• Can demonstrate how the project will be sustained after all the grant funding has been spent
• Can evidence the difference you have made at the end of the project
