1. Mapping services for Black, Asian and ethnic minority people in Wales

As part of its work for Welsh Government, EYST Wales is developing a web-based map of organisations which represent and serve ethnic minority communities in Wales in order that BAME people can then access these services. If you represent a group which is ethnic minority led or provides services specifically to ethnic minority communities, please complete this survey so that we may include it. Please only answer questions that you want to be included on this webpage. If a question is not relevant or you do not want the info included, you may leave it blank.

1. Privacy and fair usage notice relating to data: If you complete this survey, we will keep the information in a secure spreadsheet, which will be used to generate the Map webpage. This may also be shared with our funder Welsh Government. We will keep this data for the duration that the webpage is on the EYST Wales website. You may email info@eyst.org.uk at any time to update the accuracy of data, request to delete particular data or remove said organisation from the website. If EYST Wales removes the webpage, we will delete and dispose of your data.
