Introduction
The purpose of this audit is to assess compliance with the local Track and Trace procedures currently in place in the division to ensure that all movements of clinical and staff records are tracked so that they can be found quickly and efficiently when required.
Audit method
Data will be collected using SMART survey, via the on line data collection tool.
Scope
This audit applies to all areas where clinical records and staff personal files are stored and may be transferred to another service or department for any reason.
Auditors
This audit will be carried out by those suitably qualified and with a good under standing of the current track and trace procedure.
Sample size
Every team will be asked to provide a response to the audit, but results will be calculated once a minimum of 20 completed audits have been received.
