The Norfolk and Waveney Health and Care Partnership wants to make sure that people get the right care, at the right time and in the right place; including, of course, for those in the last days of their life. Good end of life care should mean that people are treated with dignity and respect and, where possible, in their preferred place of care. 

Thank you for taking the time to complete this short survey; your answer will help us make sure we have the right palliative and end of life care services in place. 

The survey is anonymous and should take about five minutes to complete. Your answers will be held securely by NHS Norfolk and Waveney CCG and in line with the General Data Protection Regulation (GDPR). A summary of the results from this survey will be shared with local health, social care and voluntary, community and social enterprise organisations who provide palliative and end of life care.
