Patient Experience Survey
What is this survey about?
This Patient Experience Survey is to help South London and Maudsley (SLaM) NHS Trust improve the care it provides. We would like you to be as honest as possible in your answers.
Why have I been asked to complete the survey?
You have been asked to complete this survey because you recently received an assessment, care or treatment from SLaM.
Do I have to complete the survey?
No you do not have to complete the survey, it is entirely voluntary. Your future assessment, care or treatment will not be affected by whether or not you complete the survey, or by what answers you give.
Is the survey anonymous?
Yes, the survey is anonymous. Please DO NOT include any personal information in the survey. DO NOT include your name or any other information that might identify you or any family member.
Who will see my answers?
All surveys are anonymous. But you should be aware that the team who gave you your assessment, care or treatment will receive an anonymised report each month. This will include any comments you have written. Anonymised reports will also be sent to key committees in SLaM that look at the quality of the care we provide.
How do I complete the survey?
If you are completing an online version of the survey, please answer all the questions you wish to answer and then click 'next page' at the bottom of the screen.
If you are completing a paper version of the survey, clearly mark the answers that most apply to you with a tick or an X. Only choose one answer unless asked otherwise. There are then two ways to return the paper survey: either hand it to member of staff, or return it via Royal Mail in an envelope marked FREEPOST PEDIC. You do not need a stamp on the envelope. Please do not write anything else of the envelope. Please ask a member of staff for an envelope if you do not have one.
