Why am I being asked to fill in this survey?
We are Healthwatch Suffolk. We ask people about their experiences of using the NHS, as well as their education, social care, and wellbeing. 
Through our research, we shape local services and make support better for everyone. 

We are working with your school, and the NHS, to ask questions about your health and wellbeing. You can learn more about us on https://healthwatchsuffolk.co.uk/.

How will my information be used?
The survey is completely anonymous. It means we will not ask for your name, or for information about how we can contact you. 

Your school or college will receive a report about the results of our survey, but staff in your school or college will not be able to know how you have responded to the survey or identify you. A report will also be shared with the public, NHS, Suffolk County Council, and local charities about the responses of thousands of other young people taking part in the survey.

What will the survey cover?
Topics within the survey include sources of happiness, how you feel about your future, healthy lifestyles and exercise, how much sleep you get, how you feel about your body image, support for mental health and other similar topics. 

You can skip any questions in this survey if you would prefer not to answer.  

What if I'm worried about my health and wellbeing?
If you have concerns about your health or wellbeing, you should try to talk to someone you trust. If you are unsure who to talk to or need support any any of the issues in the survey, you can visit our website. Go to
