Introduction
This questionnaire is for parents or carers who have been told that the local authority will not proceed with making an Education, Health and Care (EHC) plan.
You will not be asked to give any information that will disclose you or your child’s identity. It will therefore not be possible to identify anyone who has completed the survey. All responses will be confidential, and only used to support the local authority in its improvement journey. All data is stored in a password protected electronic format – please read our
privacy notice for further information as to how your data is used and your rights.
All completed surveys go into a single report to give us a view on how EHC needs assessments are managed, how we communicate decisions not to make an EHC plan and the level of confidence that children’s needs are able to be met with SEN Support.
By completing this survey we are assuming that you are giving your active consent for us to use the information you provide to help us identify what is working well and what needs to be improved.
How to answer the questionnaire
Please answer the questions in relation to your child, or the child you care for who has been turned down for an EHC plan.
There are no right or wrong answers. It is important that you answer
all questions, but you can choose not to take the survey (see question 1 below), to stop responding at any time, or to ignore any questions that you do not want to answer (after question 1).
