Transition means 'change' and is the word we use to describe the planning of a young person's future as they prepare to leave education and move towards adult life.

This questionnaire is for young people and their parents/carers to complete together and will help to inform Norfolk County Council of how it can make the transition process as smooth and person-centred as possible.
