1. Are you a parent or guardian of a child under the age of 4? Do you live in Derbyshire? If the answer is ‘yes’, we want to hear your feedback about flu vaccinations for your child/children.

 

About Healthwatch Derbyshire

 

The children's flu vaccination is available for any child aged 2-3. We want to know if you have made the decision or will be making the decision in the future when your child is eligible.

In this survey we will ask you: 

  • The area of Derbyshire you live
  • Your views on the children's flu vaccination
  • Whether your child has or hasn't had the children's flu vaccination.

We will also ask some questions about yourself. These questions are optional, but they do help us to understand how different groups of people experience health services. 

Our survey has 8-15 questions and will only take 5 to 8 minutes of your time. We will use your feedback to tell the NHS and Public Health what is working well, and what could be improved for child flu vaccinations this winter.
