Ffurflen Gais Geirda
Mae hwn yn gyfle i roi geirda ar gyfer enwebiad Medalau / Gwobrau CDdC. Am wybodaeth am ein Medalau/ Gwobrau ewch i'n wefan.
Rhaid i'r canolwr allu rhoi asesiad gwrthrychol o'r ymgeisydd neu'r tîm. Ni roddir ystyriaeth i enwebiadau heb eirda. Rhaid cyflwyno'r ffurflen hon erbyn 12:00pm 1 Mehefin 2026.
Nodwch:
- Ar gyfer Gwobrau Effaith a Medalau Gyrfa Sefydledig, mae'n ofynnol i'r canolwr fod yn annibynnol a'r sefydliad yr ymgeisydd neu'r tîm. Ni chaniateir i Oruchwylwyr Doethuriaeth na chyd-awduron roi geirdaon.
- Ar gyfer Medalau Gyrfa Gynnar (Dillwyn), mae'n ofynnol i'r canolwr fod yn rhywun sy'n gallu rhoi sylwadau gwrthrychol ar waith yr ymgeisydd ac nad yw'n gweithio'n uniongyrchol â'r ymgeisydd o ddydd i ddydd.
Ceir terfyn geiriau o 500 o eiriau. Ysgrifennwch ddigon o sylwadau er mwyn i'r Pwyllgor Craffu Medalau a Gwobrau allu asesu'r enwebiad. Gellir cwblhau hwn yn y Gymraeg neu’r Saesneg. Cewch ei lenwi yn yr iaith sydd orau gennych.

Reference Request Form
This is an opportunity to provide a reference to support an LSW Medals/ Awards nomination. For information about our Medals and Awards please go to our website.
The referee must be able to provide an objective assessment of the candidate or team. Nominations without a reference will not be considered.
This form must be submitted by 12:00pm on 1 June 2026.
Note:
- For Impact Awards and Established Career Medals, the referee is required to be independent of the candidate or team’s own institution. PhD Supervisors and co-authors are not permitted to provide references.
- For Early Career Medals (Dillwyn), the referee is required to be someone who is able to objectively comment on the candidates work and who is someone they don’t directly work with on a day to day basis.
There is a 500 word limit for the reference. Please write enough for the Medals and Awards Scrutiny Committee to assess the nomination.
This form can be completed in Welsh or English. Please complete it in the language you prefer.
