The Dementia Friendly Gurudwaras Project is a local initiative in Bradford spear-headed by Sikh healthcare professionals who are passionate about making a difference to those affected by dementia. The aim of this questionnaire is to establish the baseline understanding of dementia and its implications within the Sikh community in Bradford. Your evidence will be used as part of a campaign to design and deliver English and Punjabi dementia related workshops, courses, lectures and physical and virtual information portals.
