Arrangements for representing citizen voices in shaping our Cwm Taf Morgannwg mental health services

1. About the survey

Cwm Taf Morgannwg's Together for Mental Health Partnership Board is a partnership of public bodies and mental health service user and carer representatives.  The partnership board comes together to plan and deliver services for people needing mental health support in our area, which covers the counties of Bridgend, Rhondda Cynon Taf and MerthyrTydfil.  Everyone in the partnership understands that citizens' voices play a central role in planning and delivering our services and we want to make sure we are doing the best we can to make sure people can get involved.

This survey is aimed at finding out what people know about how things work now and how they might work best in future.  We want to hear from anyone and everyone who has in interest in making sure people's voices are heard.  This could be:

* People who need mental health support
* People who care for those with mental ill health
* Volunteers
* Service providers and their staff and volunteers
* Anyone else with an interest!

All information is anonymous unless you want to provide your name and contact details at the end – it’s up to you.  If you’d like to know more about the survey outcomes and final report, or if you have any other questions, you can let us know at the end of the survey 😊.

1. Please tell us the first part of our postcode if you know it.

2. How did you hear about this survey? *

3. Please tell us what your interest is in completing this survey (for example, do you use mental health services at the moment, feel that you need mental health support, are you a carer for someone with mental ill health, do you work or volunteer in a service that people with mental ill health come to – or it could be something else!) *

4. Do you know anything about how people with lived experience of mental ill health and their carers and supporters can get involved in shaping and changing services? *

5. What in your view is working well with the opportunities to get involved and/or represented at the moment?

6. Is there anything that needs to change about the way things work now?

7. How do you think we would be able to tell if people with lived experience and carers had been fully involved in shaping services? *

8. Is there anything else you want to say here that you think it’s important to consider in relation to people being involved in shaping services? *

9. Thanks for your time so far! This is the last question. We may want to contact some people who have completed this survey to ask for a bit more information on your answers. You may want to be kept up to date with what happens with the report. Please complete the below and if you want to be contacted, please leave your name and preferred contact method (you can leave a phone number or email address, whatever you prefer). *
