Hello,   

Inspectors from Ofsted and the Care Quality Commission (CQC) are coming to your area. They want to know about the support available for you in your local area. Your views are important to the inspectors. 
 
You can tell inspectors your views by filling out this survey. 
 
You do not have to complete a survey if you do not want to. It should take around 5 to 10 minutes to complete. 
 
You do not have to answer all the questions if you do not want to. You can take breaks. You can stop the survey at any time. You can ask someone to help you fill in the survey. 
 
We will not ask for your name or anything that tells people who you are. Please do not write anything about you or anyone else that could identify you or them.   


This survey is for children and young people with special educational needs and/or disabilities (SEND) who are between 11 to 25 years old only.

Your answers will help us to write a report and may help our inspectors on their visit. We will say what works well and not so well for children and young people with SEND. We will also say how things could be better for these children and young people. We will not write anything that says who you are when we do this.

This survey is run by SmartSurvey. You can find out what they will do with your information here: www.smartsurvey.co.uk/privacy-policy.
 
SmartSurvey will pass your information to Ofsted, and Ofsted will delete the information 3 years after the final national report is published. If we use it for other research, we might keep it for longer until that is finished. 
 
Once you have finished your survey, you will not be able to change or delete it. This is because we do not ask for your name, so we can’t tell which survey is yours.

We will keep your answers private. We do not ask for your name. We may try to find out who you are if you tell us something that makes us worried about you or another person. We will tell the local council and health services if you tell us something that makes us worried.
