We are Healthwatch Lancashire. We work to make sure health and care services are the best they can be for the people who use them.
We have heard that it can be difficult for disabled people to travel for health care, and that some places are not accessible.
We would like to ask you some questions about what it’s like for you to get to the hospital, doctor, pharmacy, and same-day walk-in centre. This could be by bus, walking, someone taking you in their car, or the hospital ambulance taking you.

We will use the information you give us to spot things that could be better.
We might use the things you say in a report, but we will not use your name and it will not be possible to identify you.
