This form is for patients experiencing symptoms of impetigo, a contagious skin infection that often appears as red sores or blisters, which may crust over.

Many mild cases of impetigo can be treated with over-the-counter antiseptic creams and hygiene measures (e.g., washing the affected area regularly). If symptoms worsen or do not improve, please complete this form for further medical advice.
