1. WORKSHOP CONSENT FORM Version 2.0, 19-Aug 2022

1. I confirm that I have read and understood the Participant Information Sheet (Version 2.0, 19-Aug-2022) sent in the introductory email. I confirm that I have been given the opportunity to ask questions and have these answered satisfactorily. *

 

2. I agree to take part in the workshop and consent to the meeting to be audio and video recorded.  I understand that all personal information will remain confidential and all efforts will be made to ensure I cannot be identified. I agree that any of the words I may use during the meeting can be used anonymously in the presentation of the research. *

 

3. I understand that the recordings of the meeting will be only accessed by members of the research team. I give my permission for these individuals to have access to the recordings and the respective transcriptions. *

 

4. I understand that my participation is voluntary and that I am free to withdraw any time prior to the workshop. However, I understand that if I withdraw after the meeting has taken place my anonymised data may still be used. *

 

5. I understand that a voting system (e.g., anonymous Zoom polls or a short, online survey) may be used to confirm group consensus or to discuss workshop topics and that the results will be stored in an anonymised format and form part of the dataset. *

 

6. I understand that all data will be stored securely at the University of Birmingham for 10 years in accordance with the UK General Data Protection Regulations (2018) after which time the data will be securely destroyed. It is possible that the named researchers for this study revisit the anonymised data during that period, for comparison purposes, which may be included in a future similar study assessing changes/innovations and regulations in hearing care and hearing devices. *
