CYFRINACHOL

Dylai’r enwebai gwblhau a chyflwyno'r ffurflen hon os ydynt wedi ateb ‘Ydw’ i Gwestiwn 2 ar y Ffurflen Dystiolaeth Enwebai, ond nad ydynt yn dymuno rhoi rhagor o fanylion cyfrinachol ar y ffurflen honno. Bydd y wybodaeth sydd wedi’i chynnwys o fewn y ffurflen hon yn cael ei thrin yn sensitif a chyfrinachol. Dim ond staff perthnasol y Gymdeithas a chadeirydd y pwyllgor craffu perthnasol fydd yn ei gweld. Bydd y cadeirydd yn asesu effaith yr amgylchiadau ar eich gyrfa, a bydd y farn honno - nid y manylion - yn cael ei chyflwyno i’r pwyllgor craffu yn ystod y cyfarfod a gynhelir i asesu’r enwebiadau a gyflwynwyd i’r pwyllgor hwnnw.

Rydym yn cydnabod y gall amgylchiadau unigol, nodweddion gwarchodedig a chroestoriad o'r rheiny effeithio ar yrfa pawb mewn gwahanol ffyrdd. Er mwyn hyrwyddo adolygiad teg, rydym wedi cyflwyno'r cwestiwn hon ac yn annog pob ymgeisydd i'w defnyddio fel ffordd o dynnu sylw'r pwyllgor craffu at unrhyw beth a allai fod wedi effeithio ar eu gyrfa a'u cyfraniadau at fyd dysg ac ymchwil.

This form is to be completed and submitted by the nominee if you answered ‘Yes’ to question 2 on the Nominee Evidence Form, but do not wish to give further confidential details on that form. The information you provide will be treated sensitively and confidentially. It will be seen only by relevant staff of the Society and the chair of the relevant scrutiny committee. The chair will assess the impact of the circumstances on your career, and that view – not the details – will be shared with the scrutiny committee during the meeting held to assess nominations submitted to that committee.

We recognise that individual circumstances, protected characteristics and their intersectionality may impact everyone’s career in different ways. To promote equitable review, we have introduced this question and encourage all candidates to use this as a way to highlight to the scrutiny committee anything that may have had an impact on their career and contributions to the world of learning and research.
