1. What is Discharge to Assess?

Are you, or do you know, someone who received care in a care home after being discharged from hospital?

After a stay in hospital a patient may need further support. Discharge to Assess services provide support to patients being discharged from hospital. There are different types of Discharge to Assess services. Some of these are arranged by the NHS and some are arranged by the Local Authority. 

 

The patient may be discharged to a care home, including residential and nursing homes, for a short period of time, usually around 4 – 6 weeks. This care home stay is to support their recovery and to assess what further care they may need.

The care offered in the care home may include therapy or rehabilitation if it is needed. This is provided by a team of health and care professionals (a multi-disciplinary team). Some people may need health or care support to continue for longer, depending on their circumstances.

 

After the Discharge to Assess stay in a care home the patient should then move to the most appropriate place for their needs. This could be returning to their own home with care and support if needed,  staying in the care home, moving to a different care home or moving to extra care housing.
