Burntwood Town Council have been working alongside local partner organisations to make Burntwood a more dementia friendly place to be. To do this we need to hear the experiences of local people who are affected by dementia.
We last undertook this survey in 2022 and it would help us to know what may have changed, so if you are living with a diagnosis or caring for someone with dementia we would like to hear your views.
This survey should take no more than 20 minutes and can be completed anonymously.

Burntwood Town Council will store this information securely and will only use it in relation to the Dementia Friendly Burntwood project.
