Outpatient Clinics Axminster Area Survey

Thank you for your interest in this survey. Led by the Axminster Medical Practice and the Axminster PPG, health providers in the Axe Valley area are seeking to improve services to provide the best possible care for our community. As part of this project we are seeking the opinions of the community.

This survey is part of that process and you have been selected because you have attended an outpatient clinic after referral by your GP in recent times and this is about your experiences of using outpatient clinics. This questionnaire has also been sent to you as a paper copy with a SAE; if you prefer, you can use that - but please respond to the survey only once.

The survey is managed by a locally market research company, Closer to Customers. In accordance with GDPR, your feedback is not attributable to you (we do not ask for your identity) and the questionnaires are destroyed within six months of the survey completion.
