This survey is being administered by Ofsted using a survey system provided by a third party, SmartSurvey. We do not collect IP addresses or any other personal information such as your name or address, but we will be able to identify your service as you will need to provide the agency's name and unique reference number (URN). This is a mandatory field so that we can analyse the data about your service. 

Any information you submit as part of this survey will be stored and processed for a short period of time by SmartSurvey. Ofsted will regularly collect submitted information. While it is held by SmartSurvey it will be protected in line with its Privacy Policy www.smartsurvey.co.uk/privacy-policy.
