Alzheimer’s Society is conducting research to understand the experiences of people diagnosed with dementia during the pandemic. We want to understand how people feel about undergoing assessments and/or receiving a diagnosis remotely.

This is a short survey of 19 simple questions about your experience with the memory service. It will take 5-10 minutes to answer. Your responses will inform our campaigning work and ensure that services meet the needs of people who are seeking a diagnosis.

This survey is intended to be anonymous so please do not provide information which would allow you to be identified. Any information obtained will be protected in line with the standards listed in our supporter privacy statement.
