We are pleased to confirm that we are in the process of arranging your initial assessment with one of our senior clinicians here at CONNECT. Before we can allocate you a date and time for your initial assessment we require that you complete your online pre-screening questionnaire. Once we have recived your completed questionnaire we will email you details of the date and time of your assessment. If we do not receive your completed questionnaire within 14 days we will assume that you no longer wish to receive input from the CONNECT service and we will discharge you back to the care of your referrer.
