In this survey we will ask you:
- Whether you live in Langley Mill, Somercotes or Marlpool
- Your views on the flu and COVID vaccinations
- Whether you have or haven't had your flu or COVID vaccinations.
At the end of the survey, you will be asked if you would like to be entered into a prize draw for a £100 Love2shop voucher. The prize draw will be done in June 2024. Please leave your contact details on the survey if you would like to be entered.
Our survey has 8-17 questions and will only take 5 to 8 minutes of your time. We will use your feedback to tell the NHS and Public Health what is working well, and what could be improved for flu and COVID vaccinations this winter.
We will also ask some questions about yourself. These questions are optional, but they do help us to understand how different groups of people experience health services.
