Thank you for your interest in joining our Trust as a member.
Becoming a member means that you can be involved in shaping services in our Trust through a number of ways. How involved you get is up to you!
You might want to join us to:
- keep up to date with our quarterly members newsletter
- share your views through online and paper surveys, and focus groups
- attend events and open days
- join a patient group work on improvement projects where we try out changes to make things work more smoothly at the Trust
- help us recruit and train our staff
In this registration form, we ask you for some personal information so that we can register you as a Trust member. There is some information that we must have to register you, and that’s marked “required” – such as your name and email address.
There is other information which is optional, but which really helps us to ensure that our members are representative of our community – such as your date of birth and your postcode.
