East Riding of Yorkshire Council Early Years Settings Coronavirus (COVID-19) Notification - Suspected Cases


This online form is to notify the East Riding of Yorkshire local authority Early Years team and Public Health of any suspected cases of coronavirus (COVID-19) relating to children or staff members.  This information will inform wider planning within the public health team in the council.

Please note, up to 10 suspected cases can be reported at a time using this form.

If a suspected case has been identified as a direct contact of a confirmed case of coronavirus (direct contact could include a parent, carer of family member):
Direct contacts must complete 14-days self-isolation from the last day they saw a confirmed case, even if a negative test is returned.
  • This is because an individual can become infectious at any point up to 14-days after contact with a confirmed case

For confirmed cases of COVID (child or staff member): 

Step 1: Inform the local authority immediately on notification of a positive COVID test result 

Owners / managers must firstly send a summary including telephone contact details into the Families Information Service (FISH) inbox: FISH@eastriding.gov.uk.  A member of the Early Years team will then make contact as soon as possible to gather further information and offer advice.  (The inbox is monitored from 9am to 8pm, Monday to Friday and 10am to 8pm, Saturday to Sunday).

Step 2: Inform the DfE
Owners / managers must then contact the DfE helpline on 0800 046 8687 and select the option for advice following confirmation of a positive case. The line will be open Monday to Friday from 8.00am to 6.00pm, and 10.00am to 4.00pm on Saturdays and Sundays.
