East Sussex Health Checks: sign-up to a co-creation group!

Social Change are currently working with East Sussex County Council to develop some creative materials and videos to tell people more about Health Checks, and we want to hear from you! We will be holding two one-hour virtual sessions to show people our ideas so far and ask for their feedback on how they could be made even better. Everyone who takes part in one of these co-creation sessions will receive a £25 voucher.

This survey is an opportunity for you to sign-up to be invited to one of these sessions. We'll be asking you some questions to help us understand who we are speaking with. Please note that all your answers will be stored securely and confidential, meaning we will not identify you at any point in the storing, processing and reporting of data.
