About this survey
Health and Social Care services in the local area have had to change the support they offer during the coronavirus pandemic. We have launched this anonymous, online survey to understand your experience of using these services during the pandemic. We don't ask for your name so it is completely confidential. If you know someone without Internet access who wants to give feedback, they can call 0118 418 1418 to book a time for us to do the survey over the phone with them.

Are you kept up to date with changes to local services?
Can you get the help and support you need from local services?
Can you find the information you need ?
If you needed to use health and social care services were they available and how did the communicate ?
What services do you really need but you can't get access ?
We are asking you to share your experiences with us, so we can ensure that those who provide the services understand how these changes are working.
We’ll share the findings with organisations so they know what you think they've done well or what they could change for the future.
