Our South West Provider Collaborative is seeking the views of young people, families and carers from across the region about current Eating Disorders/Disordered Eating services.

We want to hear from you and how you think we should transform these services within the community, home treatment and inpatient settings. It's important for us to ensure that we meet the needs of all groups in society and deliver services which offer equal opportunity for all.

This short survey is your opportunity to tell us about your experiences, both positive and negative, and to help us shape our services for the future.

Thank you.
