Vaccinations - Immunosuppressed Cohorts

 

Participant Information – Co-production Survey

 

Wandsworth Care Alliance (WCA) are currently working to investigate feelings towards vaccines/immunisations within immunosuppressed cohorts.

1. We've designed this online survey for people to do on their own. But, if you'd like to do this over the phone with someone, please email Jessica - jessica@wandcareall.org.uk to arrange a suitable time.

 

Your Data

WCA will collect your response either via an online survey or a 1-to-1 conversation online or face to face. This is only for the purpose of a report. All data will be held by WCA for 3 years and then destroyed after this time. Specific response data may be shared with the South West London Integrated Care Board (SWL ICB) but will not be shared with other third parties. All information will remain anonymised in our reports. Quotations from your responses may be included, however, we will remove any personal details that may make you identifiable. 

Participation is entirely voluntary, and you can withdraw from the research at any time, without having to give a reason. If you need to take a break at any point, please feel free to do so.
